Mission:

Boston United for Students is a broad-based coalition committed to improving the quality of the educational experience for all Boston students and teachers. We believe that there is an urgency to significantly improve student achievement so that Boston public school students graduate college ready and career prepared. We know that achieving these goals will require a fundamental change in the next Boston teachers' contract to create new working relationships and operational flexibility that will enable students and teachers to meet the challenges of public education in the 21st Century. Our drive for student success is not limited to changes in the teachers' contract. It also includes advocacy for improved administrative practices and earnest implementation of current and new contract reform measures.

PACE (Program of All-Inclusive Care for the Elderly)

Mission:

PACE, an alternative to traditional nursing home care, is an optional benefit under both Medicare and Medicaid that focuses entirely on older people, who are frail enough to meet their State's standards for nursing home care. It features comprehensive medical and social services that can be provided at an adult day health center, home, and/or inpatient facilities. For most patients, the comprehensive service package permits them to continue living at home while receiving services, rather than be institutionalized. A team of doctors, nurses and other health professionals assess participant needs, develop care plans, and deliver all services which are integrated into a complete health care plan. PACE is available only in States which have chosen to offer PACE under Medicaid.

How to get involved/application guidelines and procedures:

Eligible individuals who wish to participate must voluntarily enroll. PACE enrollees also must:

Be at least 55 years of age.

Live in the PACE service area. In Dorchester/Roxbury: Upham's Corner Health Center runs the Upham's Elder Service Plan (UESP/PACE) in Dudley Square & Savin Hill. Harbor Health also runs an Elder Service Plan in Lower Mills.

Be screened by a team of doctors, nurses, and other health professionals as meeting that state's nursing facility level of care.

At the time of enrollment, be able to safely live in a community setting.

Charge for services:

PACE receives a fixed monthly payment per enrollee from Medicare and Medicaid. The amounts are the same during the contract year, regardless of the services an enrollee may need. Persons enrolled in PACE also may have to pay a monthly premium, depending on their eligibility for Medicare and Medicaid.

Delivering all needed medical and supportive services, the program is able to provide the entire continuum of care and services to seniors with chronic care needs while maintaining their independence in their homes for as long as possible. Care and services include:

Adult day care that offers nursing; physical, occupational and recreational therapies; meals; nutritional counseling; social work and personal care

Medical care provided by a PACE physician familiar with the history, needs and preferences of each participant

Home health care and personal care

All necessary prescription drugs

Social services

Medical specialists such as audiology, dentistry, optometry, podiatry, and speech therapy

Respite care

Hospital and nursing home care when necessary

Mission:

The Bobby Mendes Peace Legacy is an anti-violence initiative co-directed by Dorchester peace activist Isaura Mendes and Shannon Flattery of Touchable Stories. It is named in honor of Isaura's oldest son Bobby who was murdered in 1995. It is rededicated to her youngest son Matthew who was murdered in 2006.

The program began in 1999 to help stem the tide of growing homicides in the Dorchester/Roxbury neighborhoods. Programs include public speaking in schools and prisons, grief counseling for families and friends who have lost loved ones, roundtable dinners with community members and their elected officials, holiday celebrations for local children and The Annual Parent's & Children's Walk for Peace.

Capeverdean Adult Day Health Center is open Monday through Friday from 8:00am until 4:00 pm. All caregivers are responsible for participants by 4:00 pm at the close of the program day. If a participant remains at the Adult Day Health Center after 4:00pm, or if the caregiver is not available, the Assistant Director will provide for direct supervision until 5:00pm. If the caregiver has not contacted the center by 5:00pm, the Assistant Director will notify protective services of the need for emergency shelter.

Mission:

Our objective is to provide social day health programs with a caring staff which enhances the quality of life for our clients and caregivers. We strive to offer social outlet that provide continued enrichment and opportunity for our elders to make new friends.

The program will provide six hours of nursing care and health supervision using an individualized plan of care. Two meal, and one nourishing snack will be prepared in accordance with the physicians’ order, and served daily. A therapeutic program inclusive of Physical Therapy, Occupational Therapy, Speech Therapy, as well as educational, social, and recreational services, will be offered as needed, to obtain and maintain the participant’s maximum level of functioning. Group and family counseling will be provided with referrals made to community agencies if appropriate.

Project Bread is dedicated to alleviating, preventing, and ultimately ending hunger in Massachusetts.

Through The Walk for Hunger, the oldest continual pledge walk in the country, Project Bread provides millions of dollars each year more than 400 emergency food programs in 128 communities statewide. Over the last four years, the organization has invested over $2 million in grants to community organizations that feed children where they live, learn, and play.

Chefs in Schools Initiative
Project Bread’s Chefs in Schools provides healthy, cost-effective meals to kids during the school day. The bill, "An Act Relative to School Nutrition," calls for meals to be evaluated for scaling up throughout the Commonwealth. The school meal program is a reliable and predictable system that provides free breakfast and lunch to the most vulnerable children in the Commonwealth. School meals are a primary source of nutrition for low-income children, providing up to 55 percent of their daily caloric intake, and the Chefs in Schools Program links good cooking with real food and serves up meals that kids like to eat. For the past four years, the program has operated in the Boston Public Schools, expanding from two to eight elementary, middle, and high schools. This year it is also expanding across the state!

Project Bread FoodSource Hotline
Project Bread's FoodSource Hotline is the only information and referral service in Massachusetts for people facing hunger. The Hotline respond to over 37,000 calls a year from people across the state struggling to feed their families. Counselors refer callers to food resources in their community and give them information about food stamps and other nutrition programs. The hotline can help families in 160 launguages - from Arabic to Vietnamese.

How to get involved/application guidelines and procedures:

All students new to the Boston Public Schools must visit a Family Resource Center with the appropriate paperwork to begin the registration process. English Language Learners are identified through a home language survey and testing provided at the Newcomer Assessment and Counseling Center. Students who meet the criteria for Newcomers Academy will be informed about the program. All students will complete a choice form to select a diploma-granting high school as well.

Students may attend Newcomers Academy for one semester or up to two calendar years, depending on the academic need, before enrolling in a diploma-granting high school. Newcomers Academy staff will continue to monitor students’ progress after they have left the program.

All students engage in rigorous coursework, including:
* Intensive English as a Second Language (ESL) and Literacy development; and
* Sheltered instruction in English, Math, Science, Technology, Social Studies, Arts and Physical Education.

Students participate in field trips and community service to support their transition to life in Boston. Instruction takes place during the school day (8:30 a.m. – 2:30 p.m.), with additional support available on Saturdays and during the summer.

Student progress is assessed through a variety of measures, including:
* MCAS exams, such as MELA-O and MEPA to assess reading, writing, speaking and listening skills;
* Math notebooks;
* Reading and writing assignments;
* Community service participation; and
* Exit portfolio.
